| Definition: | Key as determined by each agency. Individual's name (Last, First, Middle Initial). Compound last names (made up of two or more names) should be reported entirely in the last name field separated by spaces; they should not be divided into the last name and middle initial fields. If there is doubt as to whether or not a name is a middle name or a part of the last name, it should be reported as part of the last name. Suffix may follow last name in that field but the last name should not be truncated in order to include it. Special characters may not be included except in the instance of an individual having a single name (example, Cher) in which case, the single name should be entered in the last name field and a left bracket ([) should be entered in the first name field.
